Rehabilitation Process

Referral Screening
Referrals are screened to determine which model of assessment is appropriate. -
Telephone Triage
A telephone assessment based on the biopsychosocial model, after which an appropriate rehabilitation programme is recommended. -
SINA (Specialist Immediate Needs Assessment)
This is similar to an INA except that a specialist case manager performs the assessment. SINA's are utilised for complex/catastrophic and paediatric cases.
INA (Immediate Needs Assessment)
A full rehabilitation assessment is conducted face-2-face at our client's home, covering clinical aspects of the case as well as work history and social aspects. The case manager presents the information at Case Review (a consultant led multi professional panel) before a detailed report is submitted. These reports outline the rehabilitation goals, timelines and costs.
VA (Vocational Assessment)
A face-2-face assessment to determine the vocational needs and potential of our clients for a wide variety of realistic job and training options. Recommendations are outlined in a detailed report. -
Face-2-Face Programmes
This type of case management usually follows on from an INA, SINA or VA. All cases are reviewed by our consultant led multi professional Case Review panel during the life of the programme to ensure the best standards of rehabilitation.
The case management involves face-2-face meetings with the client at agreed intervals, regular phone calls to monitor progress, coordinating treatment, liaising with the NHS & community support services and monthly progress reports to all stakeholders.
We also offer specialist case management, involving the practice of a case manager who has specialist qualifications and extensive experience in dealing with clients sustaining catastrophic musculoskeletal injuries, brain injury, spinal cord injuries and paediatric cases (please refer to our Specialisation brochures for more information). -
Telephone Case Management
General case management conducted over the phone. This usually involves a concise rehabilitation report, coordinating treatment, liaising with the NHS & community support services and brief monthly progress reports to all stakeholders. -
Treatment Package
This is a rehabilitation programme package for clients sustaining low-mid level injuries. The package includes 4-6 sessions of physiotherapy or psychological treatment. -
Final Evaluation & Programme Closure
When the client has made significant progress during our rehabilitation programmes, a final evaluation is performed to assess the outcomes relating to the goals agreed in their individual programme. If all parties are satisfied the rehabilitation programme draws to an end and a summary report is issued to all stakeholders. However, it is not really the end - we encourage clients to continue on their journey to achieve their personal goals and we follow up clients to measure the effectiveness of rehabilitation after the close of the programme.
