Assessments
We assess our client’s rehabilitation needs using one or more of the following methods:
Telephone Triage:
We ask the client a series of questions covering the history of their illness or injury; the treatment they have received; their current situation relating to their pain levels; whether they are working or not, their mood and their levels of function at home, in the community and at work. These questions follow a structured format incorporating physical, psychological and social effects of their injuries. At the end of the telephone triage, a brief email report is sent to our customer to advise on the best course of action to address the identified issues.
Face-2-Face Assessment:
A client can be referred directly for a face-2-face assessment with a case manager or this can be a recommendation from the initial telephone triage. The face-2-face assessment is suggested for more complex cases and is usually conducted at the client's home. The meeting between the client and case manager covers all clinical aspects of the case; the physical and psychological impact as well as the social, work or educational issues arising from the injury. Gaining an understanding of what the client wants from the rehabilitation programme is a very important part of the evaluation process. The findings from the assessment are presented to the multi-professional Case Review (below) before a detailed report is submitted. These reports outline the rehabilitation goals, timelines and costs.
Case Review:
The Kynixa Case Review ensures that each rehabilitation programme is not only designed and monitored by the case manager but is also reviewed by a multi-professional panel of senior clinicians and vocational specialists. The panel is led by Consultants in Rehabilitation Medicine and Paediatricians and includes senior nurses, occupational therapists, psychologists, physiotherapists and vocational specialists. Case Reviews are carried out after all face-2-face assessments and a minimum of every 3 months during the rehabilitation programme, ensuring the very best of practice is adopted for each case.
Vocational Assessment (VA):
We understand that for most adults, work (whether paid or unpaid) is an important part of life and provides structure, a sense of purpose and other rewards. We assess the vocational needs and potential of our clients for a wide variety of realistic job and training options. If the client has already returned to work, we assess their vocational capabilities at their work place. We liaise with employers, government funded agencies and other specialists as appropriate to achieve the best and most sustainable outcome for our clients. Vocational recommendations are outlined in a detailed report.
